Community Living Group Homes
People Choose Where and With Whom They Live

In these homes, people receive tailored services based on their Person-Centered Plan, empowering them to live lives that reflect their unique goals and interests. This program fosters safety, autonomy, and meaningful community engagement, helping people connect, belong, and thrive.

Within the Person-Centered Plan, the following supports are addressed and tailored to meet the unique needs of each person:
Health/Medical Supports
Each individual has the right and opportunity to select physicians in their communities to address their medication and/or therapy needs. Direct Support Professionals assist the individuals with scheduling needed appointments, attend the appointments and advocating for the best possible health care oversight and treatment by the doctor, and administer medications according to the Maryland BON regulations for Medication Technicians. Nurse Case Managers/Delegating Nurses delegate the nursing function of medication administration and oversee this aspect of service delivery. Residential Coordinators track appointments and follow ups to ensure timeliness and consistency so that individuals enjoy the best possible health.
Individualized Supports and Individual Plan
Each individual’s specific needs are addressed in the individual plan, and the plan of support is tailored so that staff receive all applicable training to best support the individual. This training may include information on seizure disorders, mental health conditions, diabetes, lifting and transferring, eating disorders, special food preparation/special diets, behavioral supports needed, and special/augmentative communication used, along with other individualized training.
Recreation/Social Life Support
Individuals are supported to be involved and valued members of their communities. To this end, individuals are assisted with developing and maintaining relationships with their families and friends, participating in activities of their choosing, worshiping as they choose and in accordance with their faith, and living an active lifestyle.
Financial Services
Individuals have the right to reasonable and timely access to their personal-needs funds. Staff assist individuals with managing their funds, making purchases for items they need and desire, and with accounting of funds via documentation/receipts for purchases. Individuals will be assisted to receive all benefits (SSDI, SSI, RR, Medical Assistance, Medicare) to which they are entitled.
Home Quality & Safety
Homes will be maintained so that they are comfortable and emulate a “middle-class” environment. Homes will be personalized by the individuals residing therein and will be kept clean and safe in accordance with OHCQ and Fire Safety Code Regulations. Individuals will have their own private rooms, which they may decorate as they choose. They shall enjoy participating in the care of their own belongings (doing their laundry, cleaning their room, folding their clothes, etc.) as well as taking care of their shared belongings (assisting with clearing the dishes from the table, loading the dishwasher, dusting the living room furniture, etc.) with staff support and help.
